Location of the Pool Party If you do not see your pool on this list you cannot apply online. Please call the Bi-State Office to set up a party 636-379-3000.
Number of Guest Attending   Note: Number of guest attending means the total number of people at the party not just the number of people that will be using the swimming pool.

Will alcohol be present?  
All persons attending the party must be more than 21 year old in order for alcoholic beverages to be on the premises. No glass, only paper or plastic containers may be used.
Number of chaperones?  
For teen party (13 - 20 age group), at least one adult 25 years or older must be in attendance for every 10 teens with a minimum of 2. A printed or typed list of names, ages, addresses, and phone #'s must accompany this contract.
What time will the party start? (ie: 8:00pm)
